Last week we launched a new Care Sherpa data integration module in our Management Portal. It is the 3rd addition to the “Outreach & Engagement” category, joining integration modules for Private Health News and Conifer Health Solutions.
The integration enables an easy way for our common clients to pipe all of the Medicom Health HRA data (collected or calculated) to Care Sherpa in real-time. This real-time exchange allows Care Sherpa’s Care Consultants to flexibly use that wealth of information and insight to reach out to potential patients who have clearly expressed concern about a health condition via an HRA completion, giving an opt-in for communication in the process. The data empowers personalized and meaningful conversation in a timely manner in a way that helps patients and providers alike.
